You have been just assigned to take over a project that your management has told you is out of control. When you are asked your management what the problems were, they had no specifics, but said that the project was behind schedule, over budget, and the client was dissatisfied. Which of the following should be of the most concern to you?�
The correct answer is:
There is very little documentation related to the project.
One could be tempted to select the following answers:
- The client is very dissatisfied with the project's progress
- Your management is looking for rapid and visible action on this project to rectify the problems.

You know you are brought into a project that is out of control. You immediately start to assess what "out of control" means by asking management to identify the problems. The fact that they can not pinpoint or isolate any problem and simply state over budget and behind schedule - immediately flags to me there is little if any documentation. Without documentation, a.k.a. project plan - there is no control, no way to satisfy the client, no way to action - action what if I have no plan. Bottom line - documentation is a key process in PM and when it is not done sufficiently we are very concerned and need to action immediately. And I have been there with the boss wanting immediate action taken before we even have a chance to evaluate and analyze. Sometimes we just have to tell the bosses to give us time to react so that a long term plan can be put in place.
